What is a plan transition in the NDIS?

A plan transition refers to changes made to your existing NDIS plan. This could include a minor update (known as a plan variation), a major review (called a plan reassessment), or a complete change of support structure due to life circumstances—like moving from hospital to home, or from residential aged care to independent living.

Why might I need to transition my NDIS plan?

You may need to transition your plan if:

  • Your needs, goals or personal circumstances have changed
  • You’re moving into or out of a different care setting
  • Your current plan is ending and due for a reassessment
  • You want to access new services or adjust your funding allocations
What’s the difference between a plan variation and a plan reassessment?
  • Plan Variation: A minor change – such as updating your contact details, correcting an error, or reallocating existing funding.

Plan Reassessment: A more in-depth review that may result in a brand-new plan with updated goals and supports based on your current situation.

What happens during a plan transition?

You’ll work with the NDIA, and possibly a Local Area Coordinator (LAC), to review your needs and adjust your plan accordingly.
