FAQS

What is the NDIS?

The National Disability Insurance Scheme (NDIS) is an Australian government initiative that provides funding and support to individuals with disabilities to enhance their quality of life and participation in the community.

Who is eligible for the NDIS?

To be eligible, you must be under 65 years of age, have a permanent disability, and be an Australian citizen or permanent resident.

What does Support Coordination involve?

Support Coordination helps you navigate the NDIS, connect with services, and maximize your funding to achieve your goals.

What is the difference between Plan Management and Support Coordination?

Plan Management handles the financial aspects of your NDIS plan, including processing invoices and tracking budgets. Support Coordination helps you access the right services and make the most of your funding.

What types of individual support does SkillsConnection provide?

We offer personal care, assistance with medical appointments, mental health and well-being support, and high-intensity care for complex needs.

How does Supported Employment work?

Supported Employment helps individuals with disabilities gain meaningful work experience, develop skills, and receive on-the-job support in a structured environment.

How can I access SkillsConnection’s services?

Start by contacting us to discuss your needs and schedule an initial consultation.

Can I change my support services if my needs evolve?

Yes! We work with you to adapt your services as your goals and circumstances change.

What is a plan transition in the NDIS?

A plan transition refers to changes made to your existing NDIS plan. This could include a minor update (known as a plan variation), a major review (called a plan reassessment), or a complete change of support structure due to life circumstances—like moving from hospital to home, or from residential aged care to independent living.

Why might I need to transition my NDIS plan?

You may need to transition your plan if:

  • Your needs, goals or personal circumstances have changed
  • You’re moving into or out of a different care setting
  • Your current plan is ending and due for a reassessment
  • You want to access new services or adjust your funding allocations
What’s the difference between a plan variation and a plan reassessment?
  • Plan Variation: A minor change – such as updating your contact details, correcting an error, or reallocating existing funding.
  • Plan Reassessment: A more in-depth review that may result in a brand-new plan with updated goals and supports based on your current situation.
What happens during a plan transition?

You’ll work with the NDIA, and possibly a Local Area Coordinator (LAC), to review your needs and adjust your plan accordingly. SkillsConnection can support you through this process, helping you prepare for meetings, identify the right supports, and ensure a smooth handover if services are changing.

Can I get help with a plan transition?

Yes. SkillsConnection offers support coordination and plan management services to help you:

  • Understand your options
  • Navigate plan changes
  • Connect with new providers
  • Manage your funding during the transition
  • Maintain continuity of care
Will I lose support during a transition?

No. The NDIS is designed to ensure ongoing support. If you’re working with SkillsConnection, we’ll make sure you have a clear transition plan in place so your services continue without disruption.

How does SkillsConnection support plan transitions?

SkillsConnection helps by:

  • Reviewing your current needs and goals
  • Coordinating new or existing services
  • Supporting you through planning meetings
  • Assisting with plan management and funding during the transition
  • Working alongside your LAC or other providers
Do I need to change providers during a transition?

Not necessarily. If your current services still meet your needs, you can continue with them. If your goals or situation have changed, you may choose to explore other options, and we can help you do that.

How do I change my NDIS provider?

Changing providers is a common and straightforward part of the NDIS process. SkillsConnection can help ensure a smooth transition with minimal disruption. Here’s how it works:

  1. Review your plan & service agreement
Begin by reviewing your current plan to identify any gaps and ensure your supports still align with your goals. Check your service agreement for cancellation terms, and notify your current provider in writing (preferably via email).
  2. Find the right provider
Whether you’re new to the area or simply looking to switch, SkillsConnection makes changing providers simple. Our team will assess your current plan, identify any gaps, and connect you with the right support to meet your goals.
  3. Update & transition smoothly
We’ll help you update service agreements, handle the paperwork, and notify your provider. Our priority is to protect your rights and maintain continuity of support, so the transition feels seamless, not stressful.
What is Support Coordination?

Support Coordination is a capacity-building service funded by the NDIS to help you understand and implement your plan. A Support Coordinator works with you to connect with the right services, manage your supports, and achieve your goals.

Who is eligible for Support Coordination?

Support Coordination is typically available to NDIS participants who need help navigating and coordinating the supports in their plan. It’s often included as a separate budget item within your NDIS plan. Ask your planner or LAC (Local Area Coordinator) if you’re unsure.

Is Support Coordination funded in all NDIS plans?

Not always. Support Coordination is usually funded based on individual need. However, many plans do include it, especially for those with more complex support needs. If it’s not in your plan and you feel it should be, you can request a plan review.

Is Support Coordination in your NDIS plan?

Support Coordination is funded separately under Capacity Building Supports. If you’re unsure if it’s included in your plan or want help requesting it, we’re happy to talk it through with you.

What’s the difference between a Support Coordinator and a Plan Manager?

A Support Coordinator helps you understand and use your plan, connecting you with providers and building your capacity to manage supports. A Plan Manager takes care of the financial side, like processing invoices and keeping track of budgets.

Can I choose my own Support Coordinator?

Yes. You have the right to choose your own Support Coordinator and switch if you feel someone else would better suit your needs.

What can a Support Coordinator help with?

At SkillsConnection, our Support Coordinators can help you:

  • Understand your NDIS plan and funding
  • Find and connect with service providers
  • Coordinate multiple supports
  • Prepare for plan reviews

What is Individual Choice in the NDIS?

Individual Choice in the NDIS allows you to take control of the services and supports you receive. You choose the providers, decide how your funding is used, and set your goals. This empowers you to live life the way you want, with the supports that matter most to you.

How does SkillsConnection support individual choice?

At SkillsConnection, we put you at the center of your support plan. We work with you to understand your needs, preferences, and goals, and we provide the guidance you need to make informed decisions. Can I change my support workers or service providers?

How do I make sure my plan reflects my personal preferences?

Your plan should be a direct reflection of your needs and goals. When we work with you, we ensure that you have all the information you need to make the best choices. We’ll discuss your options, offer recommendations, and help you adjust your plan if your needs change.

What if I want to try a new service or support?

If you want to explore new services or support options, we’re here to help. We’ll guide you through the process, making sure your choices align with your goals and are covered by your NDIS plan. Flexibility is key, and we encourage you to explore services that can enhance your life.

How do I set goals for my NDIS plan?

Setting goals is an important part of your NDIS journey. We help you identify meaningful goals that align with your aspirations, whether they are related to independence, social participation, or achieving greater physical or mental well-being. Your goals will guide the supports and services that are available to you.

Can I receive support that’s not part of my current plan?

If you feel like additional support is needed, we can help you review and update your plan through the NDIS. If new supports are necessary to achieve your goals, we’ll assist you in preparing for a plan review or request additional funding if applicable.

Is Individual Choice part of all NDIS plans?

Yes! Individual Choice is a fundamental principle of the NDIS. Every participant has the right to choose their supports, service providers, and goals. SkillsConnection is committed to ensuring that you have the support you need to make the most of this flexibility.

How can SkillsConnection ensure my plan stays flexible?
